3: Detection of macrolide resistance-mediating mutations among women with mycoplasma genitalium infection in the preventing vaginal infections trial




Objectives


Mycoplasma genitalium (MG) is frequently detected in women and associated with a number of adverse reproductive health outcomes. Macrolide resistance-mediating mutations (MRMM) in region V of the 23S rRNA gene of Mycoplasma genitalium (MG) cause azithromycin resistance, the recommended therapy for this organism. We assessed the frequency of MRMM among women who tested positive for MG while participating in the Preventing Vaginal Infections trial, a randomized controlled trial of monthly periodic presumptive treatment to reduce vaginal infections.




Methods


High-risk, nonpregnant, HIV-negative women aged 18-45 were enrolled from 1 US and 3 Kenyan sites and followed monthly for 12 months, with collection of genital specimens every other month. Archived genital fluid specimens were tested for MG using Hologic/Gen-Probe nucleic acid amplification testing following the completion of the trial. Specimens that were MG+ underwent additional testing for MRMM by DNA sequencing. Descriptive statistics were used to summarize the proportion of specimens with MRMM at each visit.
