Intraoperative view of ovarian vein coils for embolization





Ovarian vein embolization is a treatment option for pelvic congestion syndrome and can be performed with coils, Gelfoam, or other embolic mixtures. In the hysterectomy of a patient with a history of ovarian vein embolization, a metallic coil-like structure can be seen along the left infundibulopelvic ligament extending from the level of the ovary to the pelvic sidewall ( Figure 1 ). The coils, rather than being confined to the intraluminal space, had eroded through the ovarian vein into the surrounding tissue ( Figure 2 ). Furthermore, the coils were dissected down to the level of the pelvic sidewall ( Figure 3 ). Finally, the coils were transected at the level of the pelvic sidewall, with the proximal coils remaining with the patient ( Figure 4 ).
