Genetic variation in solute carrier genes is associated with preeclampsia




Objective


The objective of the study was to evaluate allelic variation in 26 members of the solute carrier (SLC) gene family for an association with preeclampsia.


Study Design


Preeclampsia cases were women with mild or severe preeclampsia. Controls were enrolled from women without hypertension-related complications who presented for delivery at term (≥37 weeks). The association between preeclampsia and SLC gene single-nucleotide polymorphisms (SNPs) and haplotypes was evaluated by logistic regression models.


Results


Rs4957061 in SLC9A3 was significantly associated with a reduced risk of preeclampsia in whites (T allele, P = .002; odds ratio [OR], 0.33; 95% confidence interval [CI], 0.16–0.67). For SLC4A1 in blacks, the G allele of rs2074107 ( P = .004; OR, 1.56; 95% CI, 1.15–2.12) and the A allele of rs2857078 ( P < .001; OR, 1.67; 95% CI, 1.27–2.17) were significantly associated with preeclampsia. Also in blacks, rs10066650 in SLCO4C1 (G allele, P = .002; OR, 1.72; 95% CI, 1.21–2.46) was significantly associated with increased risk. Sliding window haplotype analyses identified significantly associated haplotypes in these genes.


Conclusion


SNPs and haplotypes in SLC9A3 in whites and SLC4A1 and SLCO4C1 in blacks are significantly associated with preeclampsia.


Preeclampsia is a disorder characterized by the onset of hypertension and proteinuria during pregnancy. Preeclampsia affects 3-10% of pregnancies and is a leading cause of maternal and fetal morbidity and mortality. However, despite decades of research efforts, the etiology of the disorder remains largely unknown. The pathogenesis of preeclampsia may be represented by a complex combination of vascular, genetic, immunologic, or environmental factors and is likely not explained by a single mechanism.


Investigation of the genetic etiology of preeclampsia has largely focused on candidate genes related to thrombophilias, folate metabolism, lipid metabolism, oxidative stress, and the renin-angiotensin system. To date, these genetic studies have not yielded consistent results and investigation of additional candidate pathways is warranted.


One such pathway is the solute carrier (SLC) family of transporters. SLC gene products transport sugars, amino acids, and inorganic ions across cell membranes. In general, transporters play a significant role in cellular homeostasis. During pregnancy, the placenta regulates the transfer of nutrients, solutes, and water between maternal and fetal blood and members of the SLC family of transporters likely play a role in this process.


Additionally, altered SLC transport in the maternal endothelium of the vasculature of the kidney and other organs may play an important role in preeclampsia. Abnormalities in SLC function in the kidney or placenta may be related to the volume abnormalities observed with this disease.


For these reasons, this study aimed to investigate whether 263 haplotype tagging single-nucleotide polymorphisms (SNPs) in 26 members of the SLC gene family are associated with preeclampsia in white and black women. To the best of our knowledge, a search of the published literature prior to February 2010 using the search terms “preeclampsia,” “gene,” and “solute carrier” revealed that no other study has investigated the role of SLC gene polymorphisms in preeclampsia.


Materials and Methods


Study population


At the Hospital of the University of Pennsylvania, the Preeclampsia: Mechanisms and Consequences case-controlled study was developed between March 2005 and August 2007. Cases were women with mild or severe preeclampsia. Controls were enrolled from women without pregnancy-related hypertension who presented for delivery at term (≥37 weeks) for spontaneous rupture of membranes, term labor, induction of labor, or cesarean section. Cases and controls were matched for self-reported race (black vs other). Institutional review board approval was obtained from the University of Pennsylvania School of Medicine prior to enrollment. Preeclampsia phenotype


Cases were identified based on prespecified maternal criteria for preeclampsia according to standard criteria of the American College of Obstetrics and Gynecology. Preeclampsia was defined as elevated blood pressure (≥140/90 mm Hg on 2 measurements ≥6 hours apart or ≥160/105 mm Hg on presentation) with new onset of proteinuria. Women who had gestational hypertension or chronic hypertension with superimposed preeclampsia were also eligible to be cases. Based on these prespecified criteria, case eligibility was determined at the time of enrollment and not by the treating physician. A total of 218 cases and 413 controls were recruited and represented 6 ethnic groups. Whites (n = 31 cases and n = 85 controls) and blacks (n = 181 cases and n = 303 controls) were evaluated in this study.


Measurements


Trained research nurses collected information on height, race, ethnicity, and history of hypertension by patient interview at the time of enrollment. Specifically, race and ethnicity were designated based on self-report. Additional medical history was collected from prenatal and hospital chart abstraction by trained research nurses. A separate individual performed data entry and all data were reviewed by 1 of the study’s primary investigators for completeness, consistency, and accuracy.


Genotyping methodology


Maternal deoxyribonucleic acid was derived from whole-blood samples, collected by phlebotomy. Specimens were stored and then processed within approximately 6 hours of collection. Samples were genotyped for the Illumina IBCv2 (San Diego, CA) array containing 50,000 gene-centric SNPs selected for their relevance to cardiovascular, metabolic, and inflammatory syndromes. From the panel of genotyped SNPs, this study focused on 263 haplotype tagging SNPs in 26 SLC genes ( Supplemental Table ). Forty-six SNPs were monomorphic in whites, resulting in a total of 217 SNPs available for analysis in this ethnic group. One SNP was monomorphic in blacks, resulting in a total of 262 SNPs available in this ethnic group.
