40 HYPOGLYCEMIA



General Discussion


Symptoms of hypoglycemia begin at plasma glucose levels of approximately 60 mg/dL, and impairment of nervous system function begins at approximately 50 mg/dL. Clinical hypoglycemia is identified by modified Whipple’s criteria: (1) CNS symptoms, including confusion, aberrant behavior, or coma; (2) a simultaneous blood glucose level equal to or less than 40 mg/dL; and (3) relief of these symptoms by the administration of glucose.


The causes of hypoglycemia may be categorized into three main groups for the purposes of evaluation. These categories are medication- or toxin-induced hypoglycemia, disorders associated with fasting hypoglycemia, and disorders associated with postprandial hypoglycemia. Each of these categories is outlined in detail below. The list of medications associated with hypoglycemia is also listed below.
