HEMATOSPERMIA

30 HEMATOSPERMIA

General Discussion

Hematospermia, or blood in the ejaculate, may result from many different causes. Most men with hematospermia are young, with infections or inflammatory disorders accounting for 39% of cases. Malignancies and trauma each account for 2% of cases. Up to 46% of cases are labeled idiopathic, while a variety of other conditions account for the remaining cases.

For the majority of patients, no significant work-up is needed, but for a minority of patients, hematospermia may be a sign of serious urologic disease.

The history should focus on trauma and infection. The past medical history may play a role if there is a history of a bleeding disorder, hypertension, or previous malignancy. A sexual history should be included. Important elements of the physical exam are outlined below.
