30 HEMATOSPERMIA General Discussion Hematospermia, or blood in the ejaculate, may result from many different causes. Most men with hematospermia are young, with infections or inflammatory disorders accounting for 39% of cases. Malignancies and trauma each account for 2% of cases. Up to 46% of cases are labeled idiopathic, while a variety of other conditions account for the remaining cases. For the majority of patients, no significant work-up is needed, but for a minority of patients, hematospermia may be a sign of serious urologic disease. The history should focus on trauma and infection. The past medical history may play a role if there is a history of a bleeding disorder, hypertension, or previous malignancy. A sexual history should be included. Important elements of the physical exam are outlined below.
