11 CHEST PAIN General Discussion Chest pain is a very common complaint of patients presenting to the emergency room as well as to outpatient clinics. More than 50% of those patients presenting to the emergency department with chest pain have acute coronary syndrome (ACS), pulmonary embolism or heart failure. However, most patients presenting to outpatient departments have diseases such as stable angina, musculoskeletal disorders, gastrointestinal disease, pulmonary disease or psychiatric disorders. The differential diagnosis of chest pain is broad and ranges from benign to potentially life-threatening. Fortunately, most cases of chest pain actually represent a much smaller list of disease processes. The history (especially risk factors for coronary artery disease), physical examination, and selected testing such as the electrocardiogram and chest X-ray can help narrow the differential diagnosis and focus further testing. Medications Associated with Chest Pain Azathioprine (pancreatitis) Corticosteroids (pancreatitis) Cyclosporine (pancreatitis) Furosemide (pancreatitis) NSAIDs (peptic ulcer disease) Oral contraceptives (pancreatitis, pulmonary embolism) Sulfonamides (pancreatitis) Tacrolimus (pancreatitis) Tetracycline (pancreatitis) Thiazides (pancreatitis) Valproic acid (pancreatitis) Causes of Chest Pain Breast pain • Abscess • Breast cancer • Breast cyst • Cyclic breast pain • Mastitis Cardiac causes • Acute coronary syndrome (myocardial infarction, unstable angina) • Aortic stenosis • Coronary artery dissection • Coronary artery spasm • Heart failure • Myocarditis • Pericarditis • Stable angina • Thoracic aortic dissection (TAD) Gastrointestinal causes • Biliary disease • Esophageal cancer • Esophageal spasm • Gastric cancer • Gastroesophageal reflux disease • Pancreatic cancer • Pancreatitis • Peptic ulcer disease Musculoskeletal causes • Contusion • Costochondritis • Disc disease (cervical or thoracic) • Fibromyalgia • Osteoarthritis • Rib fracture • Thoracic outlet syndrome • Tietze’s syndrome Psychiatric causes • Depression • Factitious disorder • Panic disorder • Somatization Pulmonary causes • Bronchitis • Lung cancer • Pleuritis • Pneumonia • Pneumothorax Only gold members can continue reading.
